Apply for a Service Dog

Our mission is to provide trained service dogs to individuals with disabilities. Returning active duty veterans and their families are our primary target population. Our dogs will be trained to provide assistance with everyday needs for those challenged by PTSD, mobility assistance, or Traumatic Brain Injury.

APPLICATION INSTRUCTIONS:

The application process consists of the following main steps:

  1. Personal Application: Complete and submit the enclosed personal application. If your personal application is approved, you will be asked to complete steps 2 and 3.
  2. Medical Application: Provide your physician with the medical portion of the application. Ask your physician to complete the form and mail it to us directly.
  3. References: Have your personal and professional references send their letters to us directly.
  4. Interview: Upon receipt and review of the above forms, a phone interview may be conducted to answer questions remaining from the applications.
  5. Home visit: A home visit and interview will be conducted if your application is considered to be a good fit for one of our dogs.
  6. Wait List: Upon completion and approval of the above process, you will be placed on our waiting list and notified when your application is within 6 months to one year of being approved for a dog.
  7. Match: You will be matched with your dog and provided with transition training in Aiken, SC.
  8. Follow Up: Follow up, home based training visits will be conducted at the time of placement and at routine intervals after placement.
